Never painted a hearth but have done a few old brick surrounds. We just used a normal semi-gloss interior water based paint.

I painted a hearth firstly with Zinnser B.I.N painter sealer from Mitre 10 which apparently gives the paint extra stickability then 2 coats of ordinary wall paint. Possibly and exterior paint could be better. It has held up well.

I have painted a concrete hearth with paving paint, it worked and lasted many years.
